Why the Cost of Spravato Can Feel Overwhelming at First

Spravato is an FDA-approved nasal spray medication made from esketamine, a compound closely related to ketamine. It's prescribed for adults with treatment-resistant depression (TRD), meaning depression that has not responded adequately to at least two different antidepressants. This affects approximately one-third of all adults diagnosed with major depressive disorder, a group that has, for too long, had very few effective options available to them.

Spravato is a relatively new and specialized medication, and at first glance, its cost can seem steep, especially for people whose insurance doesn’t cover it. That initial sticker shock stops many people before they even begin exploring treatment.

The first is Spravato withMe, a manufacturer-sponsored program that every Spravato patient at our clinic is enrolled in automatically. This program works in conjunction with commercial insurance coverage to bring the patient's medication cost down dramatically, often to as low as $10 per treatment session. For patients whose insurance covers Spravato without issue, Spravato withMe is typically the only financial tool they need.

The second is the Observation Rebate Program (ORP), which provides additional support related to the required in-office monitoring time associated with each Spravato session. Because the medication is administered in a certified clinical setting and patients are observed for two hours following each treatment, this program helps offset administration‑related costs, not the medication itself.

The third (the one we're focusing on today) is the J&J Patient Assistance Program (PAP). This program is specifically designed for patients whose insurance has denied coverage for the Spravato medication itself. And it is, in our experience, one of the most impactful resources available to anyone pursuing Spravato treatment.

What Is the J&J Patient Assistance Program and What Does It Cover?

The Johnson & Johnson Patient Assistance Program is offered by Johnson & Johnson, the maker of Spravato, that provides eligible patients with their medication at absolutely no cost. Not discounted. Not reduced. Free.

It is designed for patients who are uninsured or who have insurance that has denied coverage for their prescribed J&J medication. When a patient is approved for the PAP, J&J covers the full cost of the Spravato medication itself. That means the box of Spravato is no charge to the patient, regardless of what their insurance situation looks like.

This is a well-established, actively used resource. At Etherios Therapy, a meaningful portion of our Spravato patients receive their medication through the Patient Assistance Program. That's a testament to how accessible this program actually is and how committed J&J has been to making Spravato reachable for the people who need it most.

Who Qualifies for the Patient Assistance Program?

According to the program's eligibility requirements, patients may qualify for free Spravato medication for up to one year if they meet the following criteria:

You must live in the United States, Puerto Rico, or the US Virgin Islands and be treated as an outpatient by a US-licensed healthcare provider. You must have been prescribed an eligible J&J medication, which Spravato is, and you must meet the income eligibility threshold, which is set at or below 300% of the Federal Poverty Level (FPL). For 2026, that means a single-person household with an annual gross income up to approximately $46,950 qualifies; for a household of four, the limit rises to approximately $96,450.

Patients with Medicare Part D coverage who fall below 150% of FPL may also need to demonstrate that they are not eligible for the Low-Income Subsidy (LIS), also known as Extra Help, as a prerequisite for PAP enrollment. The program documentation walks through exactly how to verify and document this.

Importantly, the program is available to patients who have insurance, even government insurance like Medicare or Medicaid, as long as that insurance has denied coverage for the Spravato medication. The denial is not a dead end. It is, in many cases, the gateway to getting help through the PAP.

How the Application Process Works and Why It's Not as Complicated as It Sounds

If your insurance denies coverage for Spravato, Etherios Therapy begins the PAP application process as a normal next step. We think of this as a routine part of our Spravato onboarding for patients in this situation, not an emergency workaround, not a last resort, just the next thing on the checklist.

Here is what the process looks like in practice:

Your part is straightforward. You'll need to complete and sign page two of the Patient Assistance Enrollment Form, which covers your basic personal and financial information. Along with that, you'll provide a copy of your most recent federal tax return (Form 1040 or 1040-SR) and a copy of your insurance card. That's it on your end.

Our team handles the rest. We submit the completed application along with the insurance denial letter from your insurer and clinical notes from your provider. The application goes directly to J&J, which then reviews your eligibility and decides.

When approved, and the majority of our patients are approved, J&J covers the full cost of the Spravato medication itself. Your sessions at our clinic continue as planned, and the financial barrier that felt so enormous simply disappears.

What Happens If the Application Is Denied? There's Still a Path Forward

In the rare cases where an initial PAP application is not approved, we do not consider that the end of the process. J&J allows for written appeals, and our team can submit one on your behalf. In that appeal, we document the financial reality clearly, including the out-of-pocket cost of Spravato medication without assistance and what that would mean for a patient's ability to continue treatment.

We have seen very few definitive denials from J&J. Most appeals are reconsidered favorably. It speaks to what this program actually is at its core: a genuine effort by the manufacturer to get this medication to patients who need it but cannot afford it without help.
